Survey is an AMSHub Service that gives facilities the ability to send out custom surveys to patients, referring providers, or most any group they would like. It allows users to send targeted surveys to a specific list of email addresses, or create anonymous surveys that can be linked from a website or distributed to an email list. You can also send surveys to patients using SMS*. They will receive a brief text message with a link to the survey. All of the other aspects will work the same way as the traditional email delivery, but it lets you reach those patients that you have a cell phone number for but no email address.

Survey Templates
AMSHub provides some basic templates to get you started. It’s a simple process to use one of these to send a survey to a group or to customize with any changes. You can save each customized/edited version as a template in your own template library to use again any time. Learn more about this process.
